Quality Care Coordinator

Description
The Quality Care Coordinator is a key integrated care team member who promotes the mission and vision of North Country HealthCare to provide affordable, accessible, quality primary care in an atmosphere of dignity and respect. This role is vital in supporting patients to achieve their health goals through patient-centered care planning, care coordination, and community resource utilization. The ideal candidate will demonstrate a commitment to patient-centered care, have excellent organizational skills, work well in teams, and be able to adapt to the evolving needs of high-risk populations.

Duties

  • Coordinates services for empaneled patients, prioritizing those with high-risk conditions, recent hospitalizations, or poor health outcomes.
  • Uses motivational interviewing and provider-guided input to develop patient-centered care plans and self-management goals.
  • Supports health literacy and disease self-management using standardized educational materials and by connecting patients to appropriate care team members or external resources.
  • Conducts outreach to recently hospitalized patients via Health Information Exchange (HIE) to ensure safe transitions and communicate clinical concerns to the care team.
  • Collaborates with community partners and specialty providers to ensure timely, coordinated access to care and maintains an up-to-date resource list.
  • Participates in clinical team meetings, documents patient interactions in the Electronic Health Record, and contributes to ongoing quality improvement efforts.

Requirements

  • Bachelor's degree in related field or equivalent experience
  • 2 Years related experience and/or training, or equivalent combination of education and experience
  • Basic Life Support required upon hire
  • Community Health Worker Core Competency Certification/Training required within 180 days

Compensation

  • Competitive rate starting at $21.63/hr+ DOE
