Referral Coordinator

General Description

Under the supervision of the Referral Coordinator Supervisor or Referral Manager, works with the medical staff to coordinate patient referrals for medical services not provided at NCHC. Provides a variety of clerical and general office support duties to facilitate the smooth flow of patients.

Essential Function

  • Coordinates referrals to external medical providers. This duty is performed daily.
  • Obtains authorizations from various insurance companies according to their plan requirements. This duty is performed daily.
  • Tracking and follow up to close the loop making sure patients have seen referred provider. This duty is performed daily.
  • Maintains documentation and runs reports for patient referrals. This duty is performed daily.
  • Communicates status of patient referrals to the patient. This duty is performed daily.
  • Provides backup for clerical and or medical staff as needed.
  • Scanning and indexing documents to patient charts.
  • Request medical records from facilities, specialists or from portals and send them to CMR for importing.

Education

High School Diploma or equivalent GED. 

Experience
1 to 2 years related experience and/or training or combination of education and experience preferred.
