Patient Navigator

Description
The Patient Navigator facilitates breast and cervical cancer screening for uninsured, underinsured, or insured program eligible clients. They are responsible for determining client eligibility, identifying allowable services, and securing medical appointments for new and returning WWHP clients. Patient Navigator duties include monitoring and documenting screening results and case managing abnormal results appropriately.

Duties

  • Identify and mitigate screening barriers clients may encounter and assist them in completing their screenings, diagnostics, and initialization of treatment with reminders and education while ensuring Quality Core Indicators are met and/or exceeded.
  • Contact clients for new and annual services and follow-up care by phone or mail; maintain an accurate phone/client encounter log.
  • Recruit potential new and annual cancer screening participants from priority populations through referrals, mailings, phone, and/or face-to-face contact which may be either in large group and/or one-on-one environments.
  • Intake and verify program eligibility of new and returning clients. Verify timely completion and return of enrollment paperwork and assist clients with completing paperwork as needed.
  • Identify services for which each client is due and schedule appropriate medical appointments.
  • Enter data into client management database and electronic health record.
  • Track clients' appointments, screening, and diagnostic outcomes in the appropriate database and follow up with clients as needed.
  • Knowledge of clinic physicians, locations, work schedules, and appointment templates. Ensure that medical clinic staff have any client paperwork and information needed for appointments scheduled.
  • Disseminate information regarding other services provided by our organization, as well as external agencies; promote staff referrals to social service agencies that may improve quality of life (such as secure medical insurance, nutrition, and shelter).
  • Route ineligible clients to other services such as Medicaid, Medicare, Marketplace, or sliding fee services.

Requirements

  • High School diploma or GED
  • 6 months related experience and/or training
  • Knowledge of a specialized field equal to two years of college
  • Bilingual in English and Spanish preferred

Compensation
Starting at $17.36/hr+ DOE
